Visitor Health Screen Form
As directed by the Department of Education all adults coming onto the school site must complete a Visitor Health Screen form. The purpose of the form is to make sure we are looking after the health and safety of the community and will enable us to trace visitors who have been at our school should we need to. This will also allow us to comply with COVID19 Safe Workplace requirements. We have attached a copy of the form so you will know what to expect.
Examples of when we will require the form to be completed are:
- When families/carers visit the school for assemblies or volunteering
- When contractors are on site for maintenence or inspection
The form will not be required when you are dropping or collecting your children to or from school.
Forms will be kept securely for 21 days after which time they will be destroyed.
